This survey is being led by Dr. John Oliffe (email – email@example.com; phone – 604 822 7638), along with Dr. Joan Bottorff, Dr. Bernie Garrett, Dr. Michael McKenzie, Dr. Kristin Campbell, Dr. Gayl Sarbit, and Andrew Munroe at the University of British Columbia. In this survey we hope to get your feedback on the topic of exercise habits.
Some questions asked in this survey may be sensitive for some people. If you feel you may have difficulty completing this survey, you’re encouraged to discontinue now. Also, you do not have to answer any questions that you are not comfortable with, and you can withdraw at any time without penalty.
The survey should take approximately 10 minutes to complete.
