UBC Virtual Mental Health Clinic

We would like to invite you to take part in our 10-min online survey. There are plans to expand mental health services at UBC and integrate them using an online platform known as a “virtual clinic”. What services would you like to see integrated into the new platform?  Your input will help to shape the delivery and features of these new mental health services.

To recognize your time, a prize draw for a $100 UBC Bookstore gift card will be held at the close of the survey should you wish to participate, however, your completion of the survey is not required in order to be entered in the draw.  Throughout the entire study, we will respect your confidentiality. Should you choose to give us an email address, we will keep your identity confidential during and after the study.

If you decide to participate, you will be asked to consent by clicking 'I Agree' at the bottom of this page. You are free to withdraw at any time, and may choose not to participate at all, without giving any reasons for your decision and without any negative consequences. If you have any questions, feel free to contact our research team. You can reach John Ward at 604-827-3796 or Fiona Choi at 604-822-7784 or email jward@cheos.ubc.ca.

If you have any concerns or complaints about your rights as a research participant and/or your experiences while participating in this study, contact the Research Participant Complaint Line in the UBC Office of Research Ethics at 604-822-8598 or if long distance e-mail RSIL@ors.ubc.ca or call toll free 1-877-822-8598.
